Helpful Information for Patients

Checklist for New Patients

  • Schedule your first visit HERE

  • Complete your New Patient History (emailed to you when you book appointment)

  • Join Elation PASSPORT Portal - invitation emailed after booking appointment

  • You will be invited via text to Join SPRUCE for Patient Communication

Communication between Patient and Practice

  • As a Member, you have access to the Practice via direct telephone, email, text and video visits on a continuous basis, subject to the hours and limitations outlined below.

    • Non-urgent needs may be communicated by Member via the Elation Passport app or SPRUCE and will be responded to as soon as possible and within 1 business day.

    • Urgent needs will be communicated by Member via SPRUCE APP or phone call to Nurse Practitioner at 407-252-3276.  If Patient is experiencing an Urgent matter and it is between the hours of 6 am - 8 pm weekdays or 8 am - 2 pm on weekends, Patient shall text ‘URGENT’ to the SPRUCE APP text message and then call Joslyn Bear at 407-252-3276. If Member does not receive a call, text, message back from the practice after 60 minutes, Member agrees to try and call the Practice by phone again. If it is outside of these hours and care cannot wait until the next business day, please go to Urgent Care or the Emergency Department.

  • Medical Emergency.  If Patient is experiencing a Medical Emergency, Patient shall call 911 immediately or go to the nearest emergency room.

Part of the benefit of Direct Primary Care is Continuity of Care when you need it.  We try to discourage hopping on and off the membership. We also understand that life gets in the way sometimes.
